Marram Direct Debit Joining Application and Direct Debit Authority (pdf file - 220 KB) If you are an employee of ANZ NATIONAL BANK LIMITED, MERIDIAN ENERGY LIMITED, VISIONSTREAM, HP, DOWNER WORKS or an ON CALL CASUAL EMPLOYEE OF NZ POST LTD, your contributions will be paid by direct debit. Please complete the Marram Direct Debit Joining Application and Direct Debit Authority and post to Marram Community Trust, P O Box 24006, Manners Street, Wellington 6142.

Marram Payroll Joining Application (pdf file - 116 KB) If you are an employee of any other approved organisation, your contributions will be paid by payroll deduction. Please complete the Marram Payroll Joining Application and post to Marram Community Trust, P O Box 24006, Wellington 6142 or fax to (04) 385 3558.

Lifetime Declaration

Lifetime Eligibility and Declaration (pdf file - 189 KB) Download and read the eligibility criteria contained with this form to determine whether you qualify for Lifetime Status. If so, please complete the declaration and post to Marram Community Trust, P O Box 24006, Wellington 6142 or fax to (04) 385 3558.

Marram Holiday Home Ballot Form

Complete a ballot request online easily.

New ballot form (pdf file - 220 KB) Download and complete in your own time. Post, email or fax it to us before the ballot closing date. Please do not send in any older versions of our ballot form as they are not compatible with the Marram Online system and your ballot request may not be able to be entered.

Healthcare Assistance Claim Form

Claim form (pdf file - 251 KB), Download and complete a claim form to submit your claim for healthcare expenses. Please attach your original receipts and post it to Marram Community Trust, PO Box 24006, Wellington 6142. Remember to sign and provide your full bank account details.
